Track your progress

This template can help you keep track of your progress. Record your cholesterol levels in the lines provided and remember to ask for your numbers every time you get your cholesterol checked. Write down the numbers on the progress list and keep it somewhere handy, like on your refrigerator, as a reminder to keep up the good work.

Exercise plan

Keeping track of your exercise habits can help you see the progress you are making. Try listing down all the activities you do that involve physical movement, how many hours per week and a rough estimation of the calories burned. This will help you realize your level of activity and whether you are getting enough exercise.
