- You are currently a diabetes sufferer
- Or
At risk of diabetes due to:
- Age above 35 for male and 40 for female
- You have family member, whom are suffering from diabetes / hypertension /
high cholesterol
- You are overweight / obese
- You are hypertensive
- Your waist circumference >90cm (male) or >80cm (female)

Applications must be submitted via website only, upon completion of the information required.

You must search for the nearest participating Clinic / GP in your area from the list provided.

Successful registrants will be informed by telephone and a voucher will be sent by post.

You must bring along the voucher to your chosen Clinic / GP to receive your free medical screening.

The voucher is valid for 14 days from the date stated on the voucher (date of issuance).
